I, the undersigned owner or authorized agent of the above-named animal, acknowledge and understand the following:
Under Virginia Code § 3.2-6521, the owner or custodian of any dog or cat four (4) months of age or older is required to ensure the animal is currently vaccinated against rabies by a licensed veterinarian or under the direct supervision of a licensed veterinarian.
The law further states that the supervising veterinarian must issue a rabies vaccination certificate, and the owner must provide proof of current rabies vaccination upon request by an animal control officer, humane investigator, law-enforcement officer, the State Veterinarian’s representative, or a health official.
Failure to comply with rabies vaccination laws may result in legal consequences, fines, mandatory confinement, quarantine, or other public health measures as required by local or state authorities.
Rabies is a fatal disease transmissible to humans, and vaccination is required by law to protect public health.
Rabies vaccination may only be medically exempted by a licensed veterinarian when, in the veterinarian’s professional judgment, administration of the vaccine would pose a significant risk to the animal’s health due to a documented illness or a prior life-threatening adverse reaction.
